The Doctor will complete the medical report (D4) that covers

. Eyesight
. The nervous system
. Diabetes

. General health
. Cardiac health
. Medical practitioner details

You may be refused a provisional LGV driving licence if you suffer from any of the following;

. Eyesight defects, not meeting current eyesight standards
. Heart disorders
. High blood pressure (Persistent)
